Gastric Bypass Offers Clues to Diabetes Cure
Listen up all you diabetics: this kat has
become aware of some interesting new developments regarding the treatment of
Type 2 diabetes. And it doesn’t involve sticking any more needles into your
poor pincushion of a body.
Dr. Francesco Rubino of New York-Presbyterian
Hospital/Weill Cornell Medical Center has done some interesting research and
come up with compelling evidence that gastric banding may cause remission of
diabetes.
Yep, you read that right – Dr. Rubino believes gastric banding can cause
this dreaded disease, which affects 200 million people worldwide, to simply
go away.
It’s been known for some time that limiting the size of the stomach via
gastric banding helps diabetics better control their disease. This occurs
because the patient’s stomach can no longer hold the amount of food it did
before.
So, gastric banding lowers the amount of food (calories) that can be taken
into the body at one time. This leads to weight loss, which is exactly why
this procedure was developed. The relevance for diabetics is it also helps
them eat smaller meals, thereby reducing spikes in blood sugar levels.
But, and this is big, Dr. Rubino has discovered something far more
interesting about gastric banding and diabetes.
Dr. Rubino's research shows gastrointestinal banding procedures control
diabetes by bypassing the upper small intestine – the duodenum and jejunum.
Don’t those two sound just like Snow White’s eighth and ninth dwarves? But
as you can see in any diagram, these structures are actually the first
two (of three) divisions of the small intestine.
This is a key finding and may be a big step toward uncovering the origins
of diabetes. Dr. Rubino puts it this way, "When we bypass the duodenum and
jejunum, we are bypassing what may be the source of the problem."
In essence, the GI tract plays a huge role in energy regulation, and many
of the hormones in the gut are responsible for regulating sugar metabolism.
So it stands to reason that if you surgically alter the anatomy of the gut
you will directly affect how the body regulates blood sugar levels.
Simple? Actually it’s fairly complex. Dr. Rubino believes the change
in blood sugar control in diabetics after gastric banding is related to a
set of as-yet unidentified ‘anti-incretin’ factors secreted by the duodenum.
Incretins are GI hormones produced in response to nutrients as they zip
around the gut. Anti-incretin factors are thought to act as a homeostatic
counter to the effect of incretins.
For those of you not up on your biology, homeostasis refers to an
organism’s ability to stably and constantly regulate its internal
environment. The speculation is that type 2 diabetes occurs when there’s an
imbalance due to excessive anti-incretin activity. This is thought to lead
to delaying the release of insulin in response to carbohydrates, thus
raising blood sugar levels.
Now here’s the truly odd part about Dr. Rubino’s work. When gastric
bypass is performed on non-diabetic patients the result is actually
impairment of the mechanisms that regulate blood sugar. In striking
comparison, the opposite is true of diabetics; where the disease appears to
simply go away. Yes, go away, as in no need to treat patients with oral or
injected medications. This is the dictionary definition of a paradox.
It’s this kat’s opinion that Dr. Rubino’s ground-breaking research
supports our own Dr. Rodier’s position that a healthy gut is the key to
maintaining optimal health.
